This page covers the website only. The privacy of your health information as a patient of the practice is governed by our HIPAA Notice of Privacy Practices, which you receive at the office. Ask the front desk for a copy at any time.
What this site collects
Only what you type into the appointment request form: your name, phone number, and whatever else you choose to tell us. It is sent to the practice by email so we can contact you about your care. We do not sell it, and we do not share it with anyone outside the practice except where the law requires it.
Please don't send medical detail through the form
The form travels by ordinary email and is not a secure channel for protected health information. Keep it to enough for us to call you back — the clinical conversation belongs on the phone or in the office.
Location
If your browser asks whether this site can use your location, that is only to show roughly how far you are from the office. It is worked out in your own browser, is never sent to us or stored anywhere, and declining changes nothing else about the site.
Cookies and tracking
This site sets no advertising or tracking cookies. Fonts are loaded from Google Fonts, and the map on our locations page is embedded from Google Maps; both are subject to Google's own privacy terms.
